Combined Synopsis-Solicitation for Commercial Items

Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, and FAR Part 13.5 procedures, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued.
This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-1 effective 12/30/2022.
This solicitation is set-aside for Service-Disabled Veteran-Owned Business (SDVOSB).
The associated North American Industrial Classification System (NAICS) code for this procurement is 339113, with a small business size standard of 700 employees.
The FSC/PSC is 6515.
All interested companies shall provide quotations for new requirement for Boom, Lighting, and Video Systems with full integration (delivery and full installation). The required supplies and services shall be delivered and installed at SAVAHCS located at 3601 S 6th Ave., Tucson, AZ 85723.

Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Items. If paragraph (j) of the provision is applicable, a written submission is required.

All quoters shall submit all documents specified in Addendum to FAR 52.212-1 Instructions to Offerors.
SITE VISIT: In order to afford all offerors equal opportunity to observe the physical characteristics of the job site, a one-time Site Visit will take place on Wednesday January 11, 2023 from 7:00 am to 11:00 a.m. Mountain Time (Tucson AZ local time). Offerors shall meet at SAVAHCS, Building 57, 2nd Floor, Operating Room, located at 3601 S 6th Ave., Tucson, AZ 85723. The offerors are responsible for providing their own transportation to the site. The site visit is anticipated to last no more than three (3) hour. The offerors are required to RSVP via e-mail at edgar.alvizar@va.gov no later than Monday January 9, 2023 at 11:30 a.m. Pacific Site Visit RSVP Boom and Surgical Lights as the subject line.
SUBMISSION OF QUESTIONS: All questions must be received by 8:00 a.m. Pacific Los Angeles Monday January 16, 2023. All questions submitted for this solicitation must be electronically sent to edgar.alvizar@va.gov with Vendor Questions Boom and Surgical Lights in the subject line. Questions received after the stated date and time, may be addressed at the discretion of the Contracting Officer.
SUBMISSION OF OFFERS: Offers to this solicitation shall be e-mailed not later than 8:00 a.m. Pacific Los Angeles on Friday January 27, 2023 to edgar.alvizar@va.gov with Solicitation Offer Boom and Surgical Lights as the subject line. Files should be sent in PDF format and no larger than 10MB or they will be automatically rejected by the server. Vendor shall expect a confirmation e-mail indicating offer was successfully received by the Government. The Government is not responsible for rejected e-mails that exceed the e-mail capacity. Telephone and fax responses shall NOT be accepted.
Award will be based upon a comparative evaluation of quotes described in the attached solicitation (See 52.212-2 EVALUATION COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES (NOV 2021)).
