Description

Indian Health Services - Phoenix Ara Office intends to award a Purchase Order on a brand name sole source basis for the following item under FAR Part 13:





General



This scope of work describes non-personnel service requirements for ambulatory care cardiac holter monitor



services to support the mission of the Indian Health Service (IHS) by raising the physical, mental, social



spiritual health of American Indians and Alaska Natives to the highest level.





Background





The Whiteriver Indian Hospital (WRIH) is located at Whiteriver, AZ on the Fort Apache Indian Reservation. The hospital is a 40-bed facility which provides general medical, pediatrics, alcohol treatment, mental health, and obstetric services as well as ambulatory surgery. The hospital is accredited by The Joint Commission (TJC) and staffed with physicians of varied disciplines, ie Internal Medicine, Family Practice, Podiatry, Orthopedics, and Surgery as well as Nurse Practitioners, Dentists, Optometrists, and clinical nursing staff members.





The Cibecue Health Center is approximately 50 miles northwest of Whiteriver, Arizona. This health center is staffed by two physicians and one dentist, and provides outpatient, urgent care, optometry and dental services to the communities of Cibecue and Carrizo.





Whiteriver Indian Hospital and affiliated clinics serve approximately 17,000 tribal members. A substantial percentage of adults presenting for care due to signs and symptoms potentially related to cardiac disease would clearly benefit from this service which may mitigate a serious cardiac event. Though this service may be more broadly used by adult patients, the BioTel devices can also be used by pediatric patients thereby increasing the patient population that could directly benefit from this service. Initiating this service at WRIH and its’ clinics would provide local access, by providers who know the patient best, to cardiac diagnostic services that would be available with the BioTel mobile device product(s). This service would reduce the need to transfer care to a referring physicians until a diagnosis has been obtained thereby improving holistic care and reducing potential cost.





Product



BioTel Extended Holter (Epatch) – 3-14 day holter patch monitor




  • Simple patch application, enrollment and activation with no battery charging needed at practice

  • Wearable in shower, during exercise and while sleeping

  • Reports are easily accessible to providers for review anytime through the BioTel secure data portal





Mobile Cardiac Outpatient Telemetry (MCOT): 30-day continuous live telemetry monitor/event monitor




  • Simple patch application, rechargeable sensor with multiple patches for up to 30 days of wear

  • Improved diagnosis of post-stroke AF than implantable loop recorder



These mobile devices will permit WRIH providers to diagnose certain heart arrhythmias not captured during a



routine ECG test in the provider’s clinic. The device is small, inconspicuous and allows normal activities while data is captured. Monitors capture data 24/7 with “near real time” review by certified cardiac technicians that will notify the identified on-call WRIH provider should an urgent intervention be required.



This device is currently in use at Phoenix Indian Medical Center as well as Summit Regional Medical Center, a frequently referred facility which should provide seamless communication and ease of access to data for further review and assessment by Cardiology services.



Scope



The contractor, BioTel, shall provide ambulatory care cardiac holter and event monitor services, an electronic device which monitors the patient’s heart rhythm. BioTel shall provide the devices to WRIH that shall be available to WRSU patients needing diagnosis of cardiac arrhythmias. This device shall monitor and allow transmission of the patient’s heart rhythm to the patient’s designated physician to review the official report and data. BioTel shall provide notification to the physician on pre-defined criteria.



Contractor vs Hospital Duties and Deliverables




  • The contractor (BioTel) will set up the hospital (WRIH) cardiac monitoring account (user name & password) once the contract is awarded. There is no limit to WRIH users

  • BioTel will provide in-person training with identified WRIH staff

  • BioTel will provide devices/ monitors at WRIH and will continuously auto-replenish as needed

  • WRIH will enroll patients when they come into the “clinic” who are in need of a holter or MCOT/event monitor on the BioTel secure web portal. Monitor application will occur during normal clinic business hours

  • WRIH trained clinic staff/provider will place the patient on the monitor

  • Patients will wear the device home for the physician designated time frame; then return their devices back to the WRIH “clinic”.

  • Once device is received by the distribution team, BioTel will download the data and post the End of Service report for interpretation

  • If an urgent notification is auto-triggered by the patient’s device or manually by the patient, our clinical teams would contact the on-call provider at WRIH

  • BioTel shall manage all patient information in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards, Privacy Act, and IHS Service Unit and/or Health Center specific policies and protocols.

  • BioTel shall submit a proper invoice as required for payment of services rendered. A statement of service rendered will be included with a signature from the designated COR for this contract.





**If the patient needs referral to a Cardiologist at Summit Regional Medical Center or PIMC, the final report could be sent over for review and recommendation by BioTel.



The purchase of these devices will help WRSU clinic patients monitor cardiac readings and send them in in for current readings.





The vendor must have no direct interest in the project and must be a subject matter expert regarding and able to provide the BioTel Cardiac monitors with interpretation.





Firms who want to challenge the sole-source must submit an interest letter that demonstrates their firm’s ability to provide the exact equipment and rationale as to why they should be considered, as described above. No reimbursement for any costs associated with providing the capability information will be provided by IHS.

The proposed contract action is for supplies for which the Government intends to solicit and negotiate with only one source under the authority of FAR Part 6. A determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
